# Breathing and Bracing: The Valsalva Manoeuvre Explained

"Breathe out on the way up." It is the cue you hear everywhere, and it stops being true the moment the bar gets heavy. Watch anyone grind out a hard squat: they are not exhaling, they are holding their breath. Here is why, and how to do it properly.

## The principle: stiffen the trunk from within

Your spine is not a beam, it is a stack of vertebrae. Under a heavy load it needs stabilising, and much of that stability comes from the pressure you create inside your abdomen.

The clearest image is a drinks can. A sealed, pressurised can will hold a person standing on it. The same can, opened, crushes between two fingers. The metal has not changed; only the internal pressure is gone.

Your trunk works the same way. By trapping air in your lungs and contracting your abdominal wall, you turn a soft cylinder into a rigid one.

## The Valsalva manoeuvre, without the jargon

The name comes from Antonio Maria Valsalva, a seventeenth-century Italian anatomist who described it for something else entirely: clearing pus from the middle ear. It is also what you do on a plane when you pinch your nose to unblock your ears.

In lifting it comes down to three steps:

1. **Breathe into your belly**, not your chest. Your abdomen should expand to the sides and into your back, not just forward.
2. **Hold the air** by closing your throat, and brace all around as though you were about to take a punch to the stomach.
3. **Keep that pressure** through the rep, then exhale once the hard part is behind you.

What you must not do is suck your stomach in. Pulling your navel towards your spine lowers the pressure instead of raising it. Bracing means hardening outwards, not hollowing.

## Over how many reps?

The implicit rule: **the breath hold lasts one rep, not one set**. Holding through eight squat reps is the surest way to end the set because you ran out of air rather than strength.

## What bracing actually feels like

"Brace" is a useless cue if nobody has shown you what it means. Here is a ten-second test:

Put your hands on your sides at waist level, thumbs pointing back. Breathe deeply into your belly: you should feel your flanks push out against your hands. Now contract as if resisting a shove. The sides should go hard without your stomach pulling in.

That is exactly the sensation to reproduce under the bar. If you feel nothing at the sides, you are breathing into your chest and your bracing is not doing much.

## Where it applies

It does nothing for a cable triceps extension, a lateral raise or a leg extension. Holding your breath on those adds no benefit and raises blood pressure for nothing.

## What you need to know about the risks

**The Valsalva manoeuvre raises blood pressure sharply and very quickly during the effort.** In a young, healthy person this is transient and well tolerated; it is in fact what anyone does spontaneously when lifting something heavy. But it is not trivial for everyone.

**Get medical advice before deliberately using this technique if any of these apply to you: high blood pressure, particularly if uncontrolled; a history of heart disease or stroke; glaucoma or other eye conditions; an abdominal or inguinal hernia; current pregnancy or recent postpartum; pelvic floor problems.** That is not a theoretical list: these are the situations where a sudden pressure spike is a genuine problem.

Stop the set immediately and speak to a doctor if you experience dizziness, greying out, flashing lights, a sudden headache during effort, or if you have ever passed out while lifting. Those are not signs of commitment, they are warnings.

## Frequently asked questions

### Should I really exhale on the way up?

On light loads and long sets, yes, it is comfortable and harmless. On a heavy set, no: exhaling during the ascent drops your intra-abdominal pressure at the exact moment you need it most. The classic cue comes from general fitness, not from heavy lifting.

### Why do I feel dizzy after a set of squats?

It is common, and it comes from the drop in blood pressure once the breath hold ends. Exhaling more gradually at the end of a rep and not holding through the whole set reduces it considerably. If the dizziness persists, recurs often or comes with feeling faint, get it checked.

### Can you brace without holding your breath?

Yes, and on long sets you should: keep a firm abdominal contraction while taking short breaths at the top of each rep. Stiffness is slightly lower but sufficient as long as the load is moderate.

### Does a belt replace this technique?

No, it amplifies it. A belt gives your abdominals a surface to push against, which increases the pressure you generate. Without breathing and active bracing, it does almost nothing.

Breathing is an invisible variable: it shows up nowhere in your numbers, yet it explains a fair share of the sets that go badly. If you change how you brace, write it in the session note in Musku.
